2018-2019 Health Requirements Reminder

 

The State of Illinois requires a Physical Exam on all students entering the following grade levels:

  • Pre-k 
  • Kindergarten 
  • 6th grade 
  • 9th grade
  • Or entering school in Illinois for the first time

Exams must be completed within one year prior to the first day of school starting. The exam must be completed on the required “Certificate of Child Health” form. Parent signature and completion of Health History section is required on this form.

 

Any student in grades 6-12 who participate in school sports must turn in a Sports Physical and Concussion Form prior to the first practice. Students will not be allowed to participate in practice or games until these forms are turned in.

 

IESA/IHSA sports physical forms MAY NOT be used as the required school physical. However, a physical exam completed on the “Certificate of Child Health” form may be used as a sports physical.

 

 

Minimum Immunization requirements must be met including: 

  • Kindergarten immunizations: DTP/DTaP, Polio, MMR, Hepatitis B, Varicella
  • Proof of one dose of Tdap and one dose of Meningococcal is required for students entering 6th grade 
  • A second dose of Meningococcal is required for students entering 12th grade. If the first dose was given on or after the 16th birthday, only one dose is required

 

Parents having questions about physical and immunization requirements should check with their family physician.

 

 

Eye exams (by an ophthalmologist or optometrist) are required for: 

  • Kindergarten 
  • Students entering a school in Illinois for the first time.

 

Physical exams, immunization requirements, and eye exams are due at the start of the school year and must be submitted no later than October 15.

 

 

Dental Exams are required for students entering: 

  • Kindergarten 
  • 2nd grade 
  • 6th grade

 

Dental exams must be submitted by May 15 of the current school year and must be completed no more than 18 months prior to the May 15 deadline. Dental exams must be recorded on the appropriate State of Illinois, Illinois Department of Public Health, Proof of School Dental Examination form.
