Dear Parent or Guardian,
We would like to inform you about a nationwide parent survey that is administered through the Illinois 5 Essentials Survey. This is the tenth year the Illinois State Board of Education will implement this survey across the state. This survey is based on 20 years of research at the University of Chicago. The parent survey gathers information about parents’ overall relationship with the school. The information gathered from this survey will give school leaders critical input to help develop school improvement plans.
We are requesting that parents and/or guardians with a child in grades Prekindergarten through 12 complete the survey. The parent survey is available in English and Spanish and takes approximately 10 minutes to complete. To complete the parent survey, please visit https://survey.5-essentials.org/illinois/ and select Parent Survey. Surveys need to be completed by February 28, 2022. Please note that when you are prompted for the county information, it is asking for the county the district office is located in, which is Knox.
Parent/guardians’ identities will be completely confidential. Teachers or administrators will never see individual responses to survey questions and will only receive aggregated information, such as the total percentage of families who feel safe sending their child to school.
In order to generate a summary report, we need to have a 20 percent response rate from parents/guardians. We highly encourage you to take a few minutes to complete the Illinois 5 Essentials Survey to help continue the success of our schools.
